چکیده

Fetal tachyarrhythmia is a rare but serious disease. Without treatment, it may progress to fetal cardiac failure, hydrops fetalis and in utero fetal death. With the rapid advancement in fetal echocardiographic equipment and techniques, such arrhythmia can be diagnosed accurately during prenatal life by using M-mode echocardiogram and doppler. Prenatal control of the tachyarrhythmia can be achieved by either transplacental or direct fetal treatment with a wide-range of antiarrhythmic drugs. We report a 20 weeks gestation hydropic fetus secondary to supraventricular tachycardia (SVT) who was successfully treated by transplacental flecainide. Abstract Supraventricular tachycardia (SVT) is a well-recognised cause of non-immune hydrops fetalis, which is associated with a high incidence of perinatal mortality. The arrhythmia can be diagnosed accurately during prenatal life. We report here a case of fetal hydrops secondary to SVT at 20 weeks of gestation successfully treated with transplacental flecainide. The controversy on the types of drugs used and the optimal route of administration were discussed.

Review of diagnosis, treatment, and outcome of fetal atrial flutter compared with supraventricular tachycardia.

OBJECTIVE To review the diagnosis, treatment, and outcome of fetal atrial flutter compared with supraventricular tachycardia. DESIGN Retrospective review of published reports: 11 papers about fetal tachyarrhythmia published between 1991 and 2002 were selected for review. MAIN OUTCOME MEASURES All selected studies were analysed for the type of arrhythmia, degree of atrioventricular block in ...

Obstetric importance, diagnosis, and management of fetal tachycardias.

During 1980-7, 23 pregnancies of 22-38 weeks' duration were investigated for fetal tachycardia. Twelve were cases of supraventricular tachycardia, eight of atrial flutter, and three cases in which the rhythm varied between supraventricular tachycardia and atrial flutter. In 11 cases the fetus had developed non-immune fetal hydrops before referral; 12 cases were non-hydropic at referral but one ...

The Efficacy of Flecainide versus Digoxin in the Management of F E T L Supraventricular Tachycardia

Fetal supraventricular tachycardia (SVT) can be successfully treated transplacentally, but in cases where fetal hydrops develops there is considerable morbidity and mortality. The present study was carried out to establish whether the introduction of flecainide altered obstetric management and fetal outcome. A retrospective analysis took place of 51 singleton pregnancies which were referred to ...

Fetal atrioventricular valve insufficiency associated with nonimmune hydrops: a two-dimensional echocardiographic and pulsed Doppler ultrasound study.

Of 466 fetuses who underwent cardiac ultrasound examination with cross-sectional and M mode echocardiography and pulsed Doppler ultrasound and in whom we were able to follow the natural history, 12 were found to have atrioventricular valve insufficiency and nonimmune hydrops. Eleven fetuses (all of whom had structural heart disease) died either in utero or during the early postnatal period. In ...
